Daily Life Assistance
Our day-to-day life tasks can sometimes require a little extra help and direction, depending on our specific needs and circumstances. WEMA have dedicated and caring staff to assist with a range of these activities, to help you accomplish tasks, gain confidence and independence, and to live a life of our own choices. We also we liaise with allied health teams to help you achieve your goals.
WEMA can provide you with support for simple or complex daily life activities such as:

Community Nursing
Taking care of our medical needs is important for our health and wellbeing. Sometimes our needs can be complex, and require the help of a medical professional. The WEMA community nurses provide general and complex specialised nursing care inclusive of continence assessment and training, diabetes, medication, and wound care – all in the comfort of your own home.

Early Childhood Interventions
Wema Care Group is committed to strengthening communities through supporting people by offering a rare combination of services during the early childhood stages and providing resources to assist them to grow and learn. The allocated ECEI Partner will provide tools and strategies with the family to take a preventative and early response approach.

Psychosocial Recovery Coaching
We offer broad recovery and rehabilitation coaching for psychosocial disability & trauma and physical injury. The road to recovery and healing ourselves is different for everyone, and can require both short and long term management. WEMA can assist with goal orientated programs and maintenance to support you on your way.
WEMA can provide recovery based support to suit your needs including:
• Recovery Plans & NDIS Plans – we can help create plans & goals, provide step-by-step guidance, and can help you use your NDIS plan
• Coaching – providing you with motivation and emotional support, allowing you to build on your strengths and skillsets
THE ROLE OF A RECOVERY COACH:
•NDIS basics, like reporting, incident reporting, planning and plan implementation
• How to build and maintain recovery-oriented relationships to make positive change
• Coaching and mentoring skills, including goal setting and reviewing
• Crisis planning and crisis management
• Lived-experience, self-care and reflective practice
• Identifying and connecting to informal, community, mainstream and funded supports
Support Coordination
Organising our medical requirement and NDIS support can become complex depending on our personal requirements and circumstances. We can help provide guidance in understanding what is involved, and how to ensure you are getting the fullest support you deserve and need.
The WEMA team and Support Coordination services are NDIS accredited, and can help you with:
• Navigating your NDIS Plan – Assisting with any questions or concerns that you may come across.
• Choosing, Planning and Monitoring your own NDIS Plan Options – allowing you to make personal choices on where your support goes and what it covers, to suit your ambitions and goals
• Finding & Learning about Support Providers – allowing you to choose, locate and connect to other community, government and mainstream services that can assist your requirements in the best way

Social Work Services
The Social worker will offer counselling and case management including your day to day needs. This will occur by assessing you and your families needs, strengths and wishes together as well as help provide support and information to make your life easier.
WEMA offers social work services for individuals and families with options including:
• Case Management Coordination – Facilitating linkage to relevant Mainstream Services including the Health Department Centrelink & the Housing Department among others
• Counselling – Providing support for moods, anxiety as well as adjusting to illness, bereavement, and grief
• Personal & Family Mental Health – Crisis intervention for families, assistance with adjusting to major life changes and stressors
• Paediatric Health Support – help with therapy options for children that are relevant to their age group
